ENROLLMENT INFORMATION FOR FALL 2010

KINDERGARTEN - INFORMATION
KINDERGARTEN REGISTRATION PROCEDURES…Registration for all kindergarten students
will be held at
FIRST DAY OF SCHOOL…Classes
will begin for all kindergarten students on Tues., August 17th at 8:30
a.m. Dismissal will be at 10:00
a.m. All students will stay at school
until dismissal. Parents are asked to
bring their children on the first day.
AGE FOR ENTERING SCHOOL…Kindergarten students must be five years old on or before September 1,
2010, to register.
DOCUMENTATION NEEDED FOR REGISTRATION…Parents need to bring the following for
kindergarten registration: Proof of
Residency, Official Birth Certificate, and Physical /
Immunization Records (See General Registration Information - All Grades).
GRADES 1 - 8
INFORMATION
GRADES 1-5 REGISTRATION PROCEDURES…Registration for all students in grades 1-5
will be held on Thurs., August 12th between 12:00 noon and 7:00 p.m. at
*NEW* GRADES 6-8 REGISTRATION PROCEDURES…Registration for all students in grades 6-8
will also be held on Thurs., August 12th between 12:00 noon and 7:00 p.m. at
Centralia Junior High School
FIRST DAY OF SCHOOL…Classes
will begin for all students in grades 1-8 on Tues., August 17th at 8:30
a.m. Dismissal will be at 11:00
a.m. All students will stay at school
until dismissal. Parents are encouraged
to bring their children to school on the first day.
GRADE 1-5 ATTENDANCE CENTERS…All grade 1-3 students will attend either
Students in grade 4 will attend Irving School.
Classes begin at 8:30 a.m. and dismiss at 3:00 p.m.
Students in grade 5 will attend Lincoln Fifth Grade Center.
Classes will begin at 8:30 a.m. and will dismiss at 3:00 p.m. on regular
school days. State law requires that
students provide Immunization Records upon enrollment into grade 5.
P.E. UNIFORMS…All 7th and 8th grade students are required to
purchase P.E. uniforms for $12.00 payable the first day of school.
GENERAL REGISTRATION INFORMATION -
ALL GRADES
DOCUMENTATION NEEDED FOR REGISTRATION…Parents need to bring the following
documentation to registration.
1. Official Birth Certificate - Required only of
kindergarten and students new to the district.
2. Physical / Immunization Records - Required
only of kindergarten, grade 6 and in-state transfers.
3. Immunization Records Only - Grade 5
4. Proof of Residency - Required only of students
entering school for the first time, students new to the district or students
who have moved within the district.
OFFICIAL BIRTH CERTIFICATES…Copies of birth certificates issued by
counties or municipalities will be required
of all students entering kindergarten and new to
the district. Birth certificates
issued by hospitals will not be accepted.
PHYSICAL AND IMMUNIZATION RECORDS...Completed physicals with physician signature
and immunization records as required by law must be provided at registration
for all students entering kindergarten, grade 6 and in-state transfer
students. Dental exams will be required
of all students in kindergarten, grade 2 and grade 6 before the end of the
school year. Vision exams will be
required of all students entering kindergarten and public school for the first
time. Centralia City Schools has a very
strict policy for head lice infestation.
The School Nurses will conduct head lice checks early in the school
year.
RESIDENCY...Documentation of residency from each of the two following categories
must be provided at registration by parents of students entering school for the
first time, students new to the district or who have moved within the district.
Category
1: Tax
bill, deed, two rent receipts, signed landlord statement confirming residency, or
notarized home AND
owner statement verifying residency at the home owner’s address
Category
2: Recent
bill or driver’s license confirming residency
TRANSPORTATION...Transportation services will begin on the
first day of student attendance, Tues., August 17th. Questions concerning bus routes may be
directed to your child’s school, or to West Bus Service at 532-0431. Please have your child at the bus stop early
in case the bus is ahead of schedule.
Routing times will become more reliable after the first few days of
school. Parents of special education
students will be notified by letter or phone concerning transportation service
for their children.
TRANSFER STUDENTS…In-state transfer students must provide an
official birth certificate, current health/immunization records at
registration. Out-of-state transfer
students must provide health/immunization records within 30 days of enrollment.
BOOK RENTAL ...There will be no book rental fees for the
2010-2011 school year.
SCHOOL SUPPLIES...A list of all needed supplies will be
provided at registration. Stores will
also have school supply lists.
SCHOOL MEALS...Beginning the first full day of school,
Wed., August 18th, lunch and breakfast will be served at all attendance
centers. Lunch will be $2.25 per day and
breakfast will be $.75 per day. Some
students will be eligible for free or reduced price meals depending on family
size and income. Application forms will
be available at registration.
EARLY DISMISSAL...Due to the usual hot weather, school will be
dismissed early through Friday, September 3rd.
Classes at the
STUDENT INSURANCE...All K-8 students are automatically enrolled
to receive free, limited accident insurance coverage against accidents while
participating in school-sponsored events.
Parents will receive specific information concerning this benefit. Twenty-four hour insurance may be purchase at
an additional cost.
LATE REGISTRATION…Students not registered during their
respective registration times may be registered for school at the buildings
which the students will attend beginning on Tues., August 17th, between 1:00
p.m.- 4:00 p.m. and after school thereafter. No
students will be registered between 8:00 a.m. and dismissal time. Students
will not be enrolled in school until they are registered.

POLICY...It is
the policy of Centralia City School District No. 135 that it does not
discriminate on the basis of sex in the provision of programs, activities,
services or benefits. Centralia City
School District No. 135 guarantees both sexes equal access to educational and
extra-curricular programs and activities.
ASBESTOS INSPECTION AND MANAGEMENT PLANS...The Centralia City Schools Bd.
of Ed. has had asbestos inspections conducted of all district owned facilities
as required by the Asbestos Hazard Emergency Response Act (AHERA) and Asbestos
Management Plans prepared for all attendance centers. The Inspection and Management Plans have been
submitted to the Ill. Depart. of Public Health as
required by law. Copies are available
for public inspection during regular hours at the Administration Bldg., 400 S. Elm,
or at attendance centers by appointment with at least one working day advanced
notice. Initial cleaning as required by
AHERA was completed in all Dist. No. 135 attendance centers during the summer
of 1989, with 3-yr. inspections conducted thereafter as required by law. The most recent 3-yr. inspection was July,
